while the vessel was at sea, 3/Off was smoking on the bridge (A NON-SMOKING AREA) and later, he had put the cigarette bud in the dustbin containing waste *****s. The cigarette bud was not properly extinguished. He realized his mistake when the *****s in the bin started smoldering and there was a little smoke on the bridge. Same was extinguished immediately and there was no damage whatsoever.

while carrying out welding job on engine room stbd side blower,foreign particle entered Peon's left eye and same was remaining in the cornea.washed the eye with water and eye wash,however the foreign particle was still seen in his eye.continued with eye washing regularly until vsl reached port Contrecoeur where he was refered to the doctor and the foreign particle was removed.

2/E and 3/E renewed M/E fuel oil hot filters. While filling up the filter chamber with fuel oil from in use filter, pipe union broke at base and hot oil spilled onto 3/E causing superficial burns.-10

Non relevant persons not to visit the mooring stations/areas, during mooring operations. Same to be strictly ensured by the respective officer-in-charge.

During this time, the crane traversing control wires became entangled with the hoisting chain and not noticed.
The swing valve caught the chief engineer in the chest/ abdomen area and pushed him backwards against A/E No.2. as the valve then swung away from the C/E he collapsed to the floor. C/E was given to Cardio-Pulmonary resuscitation。
