We have been informed by one of our esteemed principal regarding a multiple-fatal incident due to accidental release of CO2 which occurred onboard their vessel.
The unfortunate incident occurred on a container vessel during docking in <st1:country-region w:st="on">China. Please go through the attached CASUALTY REPORT and discuss with all staff onboard during the safety committee meeting.

SUBJECT: A Fatal Incident Caused by the Accidental Release of CO2
Most regrettably, a fatal incident occurred on a container vessel operated by NYK while the
vessel was dry-docking. You are kindly requested to review the below report and take the
necessary precautions to avoid any similar incident.
<Summary>
While a container vessel was dry-docking in <st1:country-region w:st="on">China and a crewmember was doing a blow of
the CO2 line for the CO2 fire-extinguishing system, CO2 was released into the engine room
by accidental operation of the CO2 release device. As a consequence, three members of
the engine crew, who were working in the engine room at that time, were found
unconscious in the engine room and later confirmed dead at the hospital. Several other
crewmembers and one dockhand were hospitalized.

You are kindly requested to follow the below points to avoid a similar accident.
Before the start of work each day, the crewmembers, superintendent, 1. and dockyard
workers should confirm the content of work (including all related safety measures) and
schedule for the day; thereafter, everyone should follow the work schedule and
implement the safety measure carefully. In order to perform the above procedure
thoroughly, it is very important to conduct a proper pre-meeting.
2. The progress of work should be confirmed point by point with all concerned parties.
3. The preparation of high-risk work, including the confirmation of the proper safety
measures, should be conducted before work is started.
4. The high-risk work that was discussed in the pre-meeting should be carried out under
supervision of the officers who are in charge.
In addition to the above items, all crewmembers should remain familiar with the structure,
operational procedure, and maintenance of all equipment and systems on board, such as
the CO2 fire-extinguishing system for this case.
