4th Engineer lost tip of index finger.-11
1.Master to foward more detailed report in order to properly analyse the Incident/Near Miss.
2. Incident to be discussed in safety meeting and probable cause to be established.
A/E No.3 Maintenance was being carried out. While replacing Cylinder head of unit No.8, 4/Engrs right hand index finger got stuck in between cylinder head and push rod cover. Resulting in cutting off tip of his index finger. -12
1. Senior staff to supervise overall Operation and if required Risk assessment to be carried out prior commencement of Work in spite of same work description included in Risk Assessment Matrix.
